Job Overview: 

The Behavioral Health Medical Assistant/Population Health Care Manager is responsible for supporting clinical staff as assigned in the Behavioral Health Department under the direction of a Psychiatric Nurse Practitioner and perform administrative and clinical duties under the Health Informatics Technician/Quality Improvement Data Analyst. 

Population Health duties include assisting with the overall strategic coordination of quality improvement, population health and care management programs.  Serves as an instrumental team member for the quality and population health program, providing strategic vision and direction to all clinic departments to ensure BCHC’s transition to implementing innovative models of health care delivery, and execution of quality and population health strategies. These strategies are intended to lead to high quality, patient-centered coordinated health outcomes for BCHC patients. Works with the Health Informatics Technician /Quality Improvement Data Analyst to ensure population health programs coincide with standards set forth by PCMH, UDS, Medicaid, Medicare, and commercial insurances.  Help maintain BCHC’s FQHC quality program to ensure compliance with internal QI initiatives, FQHC standards, and other government programs.

Clinical duties may include taking and recording vital signs and medical histories, preparing patients for examination, entering patient data and information into the electronic records management system, collecting, and analyzing performance measure data to ensure compliance.  Deal with sensitive information and maintain strict confidentiality. 

Essential Functions (Major Duties or Responsibilities): 

Clinical essential functions include reviewing patient medical records prior to appointment and updating the behavioral health provider as to the needs of each patient.   Prepare treatment rooms for patient examinations.  Interview patients to obtain mental health and medical information and measure vital signs.  Dispose of biomedical waste in accordance with standards.  May perform venipuncture for labs depending on qualifications.  Record patients’ medical history, vital statistics, or information such as test results in an electronic medical record software.

Population Health essential functions include running and analyzing reports using electronic medical record software and/or other analytical software to retrieve, edit, and tabulate data from databases.  Gather, review, and analyze patient health status, and access data from patient databases, identify by type, check for completeness and compliance with established criteria, obtain any missing information and resolve discrepancies before entering them into a computerized database. Update data in the database to provide accurate and reliable reports.


Minimum Qualifications (Education and Experience): 

Medical Assistant Certificate or comparable previous work-related skills required, knowledge or experience is preferred.  Experience in clinical reporting and analysis is preferred.

Knowledge, Skills and Abilities (KSA’s):

Knowledge of principles and processes for providing personable customer service.  Includes patient needs assessment, meeting quality standards for services and evaluation of patient satisfaction. Knowledge of medications, immunizations, communicable diseases, CPR, and other related nursing procedures.  Active listening, good communication skills and social perceptiveness.  Ability to work independently and as a team player.  Ability to meet the standards and metrics of the patient centered medical home model and quality improvement measures.  Strong user of Microsoft Word, Excel, and PowerPoint.  Basic skills in healthcare clinic operations and medical terminology preferred. 

Physical and Environmental Demands:

Work is performed in an office and clinic setting; stands, walks with intermittent sitting; reaches for and uses writing instruments and keyboard; reads reports and other written materials; extensive use of telephone and oral communication with the public and coworkers; stoops; bends; kneels; reaches for; picks up; and pushes or pulls; ability to lift up to 30 pounds.

Special Requirements: 

Performs duties in the deliverance of health services.  Hazardous risks may include exposure to infected body fluids, sharp instruments, and chemicals, requires adherence to universal safety precautions.

Team Approach: 

Managing patient care is a team effort that involves clinical and nonclinical staff (i.e., physicians, nurse practitioners, physician assistant, nurses, medical assistants, schedulers, billers and front-end staff) interacting with patients and working as a team to achieve stated objectives.   Emphasis is on ongoing interactions of team members to discuss roles, responsibilities, communication, and patient hand-off, working together to provide and enhance the care provided to patients.  All staff are members of the team.  Involvement of the patient/family/caregiver with care team members is critically important to patient-centeredness.
